A leg ulcer is defined as a loss of skin below the knee, which takes more than six weeks to heal[1] [Fig 1]. Venous leg ulcers are caused by sustained high pressure within the venous system of the leg. Over half are caused by progressive venous reflux that begins as varicose veins, and the remainder develop after a deep vein thrombosis[2]. The key to effective management is accurate assessment of risk factors to enable the appropriate diagnosis to be made and the use of compression therapy to reduce the venous hypertension. In order to achieve this there are a number of important factors to consider.

Skin disease such as venous eczema and tinea pedis are often overlooked as primary causes of lower limb cellulitis. This article will look at the management of these two common skin conditions as well
as the diagnosis and management of lower limb cellulitis. Kilburn et al (2003) acknowledge that the
treatment of cellulitis is multiciplinary stating that no one speciality can claim the condition as their field and the Clinical Resource Efficiency Support Team (CREST, 2005) recommend that key staff in an integrated cellulitis pathway should include dermatologists. The article will give details of a dermatologyled lower-limb cellulitis service which demonstrates a best practice initiative which avoids unecessary
hospitalisation, incorrect diagnoses and also helps to reduce the risk of recurrent cellulitis.

This on-demand version of a live webcast broadcast on 8 June 2011 focuses on the problem of wounds where healing is prolonged or never achieved. Appropriate referral of these patients with access to the right treatment at the right time to manage patients' symptoms is key to achieving good concordance and in reducing the associated costs. These goals were recently highlighted in a study from the US that assessed the efficacy of a topical antibacterial dressing in patients with infected, hard to heal venous leg ulcers. The results offer a interesting benchmark for clinicians.
